The Southern Crescent Area Agency on Aging contracts with the Georgia Division of Aging Services to provide programs to the elderly.  The Southern Crescent Area Agency’s vision as adopted from the Division of Aging Services is: “Older Georgian’s Living Longer, Living Well.”  Our mission is to promote independence and well being for older persons in our region by providing quality programs and services.  We strive to provide the best services possible and in order to do this, we need your input on the quality of services.

 

Please answer the questions in the survey below.  There is also a place you can provide further comment if you would like, however, this part is optional.

 

Only truthful answers will help us make improvements in services.  No matter how you respond to any of the questions, your current service WILL  NOT be affected in any way.  We respect your privacy, therefore we are not requesting names, addresses or other personal information.  Your response will be kept anonymous.
