What is Intake and Screening?
Intake and Screening is the process of determining need and eligibility through a guided series of questions.
 

Intake and Screening is a service for older individuals that provides current information on opportunities and services available within the community, including information relating to assistive technology; assesses the limitations and capacities of older individuals and links individuals to opportunities and services available.

 

To the maximum extent practical, Intake and Screening staff ensures individuals receive the services needed, and are aware of the opportunities available by establishing adequate follow up procedures.

 

The Intake and Screening program assists with promotion of independence and continued well being of the areas older adults.

 

The Intake and Screening process also assists in the development of a comprehensive and coordinated system of services designed to assist older members of the community to achieve healthy, independent and self sufficient lives.

 

The Southern Crescent Gateway Staff will respond to your requests by identifying need, determining eligibility, identify providers able to meet your needs and make referrals with a goal of making your call to us the only one you need to make.
